To request a certification, please follow these steps:

 

1. Fill out the Request Form for student documents. Submit the form to the Registrar's Office clerk for assessment of fees.

3. Present the original document (e.g., original Transcript of Records)

3. Pay the corresponding fee at the Cashier’s Office.

4. Submit the Official Receipt, completed request form, and required documents to the clerk in charge.

5. Receive the claim stub indicating the date and time of release.

6. Present the claim stub and a valid ID when claiming the document. If someone else will claim it on your behalf, an Authorization Letter or Special Power of Attorney (SPA) must be presented.
