If your Student ID is lost, damaged, dilapidated, or unreadable, you may request a replacement.
In Article 33, Section 123 of the 2017 Revised Student’s Handbook of Eastern Visayas State University (EVSU), the following procedures are provided:
Steps to Replace Your Student ID
Step 1: Present your Certificate of Registration (COR) at the EVSU Printing Press Office. You will be given a Request for ID Replacement Form.
Step 2: Fill out the Request for ID Replacement Form.
Step 3: Pay the required ID replacement fee at the EVSU Cashier’s Office. Present your Official Receipt to the Printing Press Office.
Step 4: Proceed with your picture taking at the Printing Press Office.
Step 5: Wait approximately 5–10 minutes for processing and printing, then claim your new Student ID.
Important Reminder:
->> Always keep your Student ID secure, as it is your official identification inside the University.
