If you are planning to transfer to another institution, you must request your Transfer Credentials.
This includes:
- Honorable Dismissal
- Certificate of Good Moral Character
- Informative Copy of Transcript of Records (TOR)
->> This process is allowed only once.
Steps to Request Transfer Credentials:
- Accomplish a Student Clearance Form from the Registrar’s Office.
- Fill out the Application for Issuance of Student Documents and check the box for Transfer Credentials. Submit it to the Registrar’s clerk for fee assessment.
- Pay the required fee at the Cashier’s Office.
- Submit the complete requirements to the Registrar and wait for your Claim Stub (with release date and time).
- Present your Claim Stub and valid ID when claiming your documents.
->> If someone else will claim it, they must present an Authorization Letter or Special Power of Attorney (SPA).
